Pirfenidone

Pirfenidone is used to treat a lung disease called idiopathic pulmonary fibrosis (IPF). IPF causes scar tissue to form deep in your lungs. Scar tissue hardens or thickens over time, which can make it harder for your lungs to function.

Decreased lung function may make it difficult for you to breathe. When the brain, heart, and other organs do not receive enough oxygen, other diseases are likely to occur.

Pirfenidone is an oral small molecule that inhibits collagen synthesis, reduces the production of cytokines, and inhibits the proliferation of fibroblasts and stimulation in response to cytokines. It is effective in lung, kidney, and liver fibrotic conditions.

Mechanism of action

Pirfenidone is an orally active, small-molecule drug with a wide range of biological effects. In laboratory studies, it inhibits collagen synthesis, reduces profibrotic cytokines, and reduces the proliferation of fibroblasts. By reducing gene expression, pirfenidone reduces the amount of TGF-beta2 mRNA and its mature protein and directly inhibits the furin convertase enzyme that processes the protein precursor of TGF-beta2.

In addition, it reduces matrix metalloproteinase 11, which is the target gene of TGF-beta and furin substrate involved in carcinogenesis.

Method of Use

Adults:
Idiopathic pulmonary fibrosis (IFP):
Liver function test should be done before starting the drug.
Take the medicine with food.
Day 1-7: 267 mg (one capsule) orally three times a day
Day 8-14: 534 mg (two capsules) three times a day orally
Day 15 onwards: 801 mg (three capsules) orally three times a day
Maintenance dose
801 mg (three capsules) three times a day orally with food
